Add a Survey Widget to the Right Column

Change the Height to Stretch
The Survey Widget occupies the entire height of the Column

Create new survey
Name = Adopt a Road Survey
Tags = Maricopa
Create
This launches the Edit Survey Window
Select Singleline Text as your first question.
Edit the Label
Label = Road Name:
Scroll Down
Make this Question Read-only
The Road Name will be passed from the Map to the Survey when you select a Road
Open the Add Tab
Select Singleline Text
Label your 2nd Question = From Road:
Make the 2nd Question Read-only
Select Singleline Text
Label your 3rd Question = To Road:
Make this 3rd Question Read-only
Select Map
Label = Road Location:
Very Important!
Select Line
The Road that you select on the Map will be drawn on the Survey as a Line.
Set the Initial Extent
Zoom in close to the City of Phoenix
Now, zoom in even closer to a Street in Phoenix
This sets your initial Zoom Level
Next, you are going to ask for the First Name, Last Name, Email and Phone Number.
Select Singleline Text
Label = First Name:
Make this a required question.
Select Singleline Text
Label = Last Name:
Make this a required question.
Open the Appearance Tab
Select the Blue Theme
Turn off the Survey Description
Turn off the Survey Footer
Save your Survey
Preview your Survey
Check how your Survey will appear on a Phone
Close the Preview
Publish your Survey
This takes you back to the Experience Builder
Turn on the Survey Header
Enable Survey Theme
